Wiring problem with icecap 660?

    I have a icecap 660 that I'm trying to hook up. I'm running 2 - 4ft bulbs. I followed the diagram here for a 2 bulb configuration. When I plug the ballast in, the bulbs flash for a split second and goes out. I've changed out the endcaps and triple checked all the connections. I'm out of ideas, does anyone have any suggestions?
