Cupramine IS effective against velvet. If the spots don't have a yellowish or gold hue, then it's most likely ich. The fish has not been at a full therapeutic level for very long. It would be best to stick with copper rather than try a new treatment at this point.

What level have you maintained, and for how long now?

I have slowly been raising it to .6ppm as of today. I have maintained .5 ppm for two days prior with little if any improvement. I do think it is velvet. I could be wrong but its what I think.

With a bad infestation, that's not quite enough time to see a remarkable improvement. If it is really velvet, chances are he could have died already. Just need a little patience.

And if it is velvet which is what it is I believe it is and I sit around and wait too long I could lose the fish. I have no way to do a scraping but millions of tiny white spots making the fish seem to have a second skin sounds like velvet to me.

But Cupramine is effective against velvet. So you are treating the fish correctly either way.
