i would say its water quality

ive had toads under all types of lighting and flows and never had any bad experiences..

water quality is the first thing i would check..

theres really nothing different with my water, at least with the main parameters i test for, Ive done 5 gallon water changes every week ever since ive had it set up. I still test every once in a while but usually not unless something seems wrong. I tested about almost 2 weeks ago and nothing was wrong and havent done anything different since then. and all my sps are doing great.

when it is open the polyps sway back and forth pretty good so it keeps debris off of it but its not blowing hard enough to make the whole toadstool sway very much at all. ive messed with the flow some but it hasnt helped

    are you running carbon?
any chance that it may be irritated by its neighbours?

yes I am running carbon, i think you were the one who told me about running carbon with leathers and sps so thanks for that. as far as it being bothered theres only a purple plating montipora a few inches away from it and its not able to make contact with it at all. It is as high in my tank as I keep my sps corals but but i dont think that my lights would be the problem. Im just running 4 t5 hos over a 40 gallon

Mine has done the same thing maybe 4 times in the year that i've had it. It always comes back but i never found out what causes it, from what i've read people say it happens and not to worry. Mine's been shedding for the past couple of days so hopefully it'll come back to life soon. I'll be watching this thread to see if anyone does have an answer, good luck.
