    About a week ago he had a bugged out eye that went away but now his lips and the right side above the lateral line is showing white patches, is this hole in the head??

    It is hard to see the spots on his head but you can see the lip, if you go to the image on myfishpix.com you can get a larger image

    Often times bacterial infections are behind popeye. Do you have a UV? Could be lateral line erosion, but its not all that common in pseudochromis as far as I know. Its more common in tangs and herbivores because it's harder to get them proper nutrition on captivity.

    I would put my money on a bacterial infection. Would use wide spectrum ani-biotics in a hospital tank, or melafix dips or melafix dosing in a hospital tank
