    Lets see...
    1) pearly jawfish a few years back
    1) Green spotted puffer a few months ago
    1) green chromis a few weeks ago

    the jaw I can see especially when new but the other 2 still surprise me

    About 10 years ago my snowflake eel made it to the carpet. I found my cat curiously pawing it. Threw him back in, and he made it just fine. Pearly jawfish a few years before that, never made it. My yellow coris wrasse about 8 months ago--died. My green mandarin about 6 months ago.

    It's a bad problem. I won't cover my tank, and just don't put "jumpers" in anymore. That's why I was surprised about the mandarin and the coris wrasse. They're not known to be jumpers.

    I had a Coral Beauty, I think my Sailfin caused it to jump..

    the hood has saved mine many of times
    sometimes i find one of my fish in the fuge cause they jump hit the hood and fall into the back chambers

    although when i was switching the sand
    when they where in buckets, 2 of them jumped out onto the carpet was only for a moment though

    Never. I have a glass lid on my tank. Sometimes they do make it into the overflow though.

    We had a pinkbar goby jump, which was very tragic because he was paired with our Tiger Pistol. However, he was replaced by a yellow watchman and I like the YW goby much better :D

    Second was our beloved Coral Beauty who jumped at the end of last may. She was my mom's favorite, and it was a very sad day. A pygmy angel and purple firefish took up the bioload left behind :) We have since then made a night time cover made of egg crate and a mesh of some sort. It allows the tank to "breath" at night, but we keep it off during the day, because the area in front of our tank is such a high traffic area in our house a fish has a good chance of being saved if it jumped in daylight. Plus, our two jumpers happened at night.

    True perc jumped and made a fish chip.

    I bought a pair of blue/green chromis for the wife a few months back. Jumped out of the tank at the LFS when they tried catching him. He lived, at home once I came up to the tank too fast and he jumped into the MH (which had an exposed bulb at the time and was running for 4 hours before) and got burned pretty good. Hes alive and looks great though... the little buggers :)
