How old are your builds? Have you tested phosphates?

Lack of surface disturbance allows the nitrifying bacteria to bond your sand grains into clumps. The clumps can restrict aeration to the lower layers causing dead zones where toxins can build up. A couple of Nassarius snails will take care of that for you.
