it is hard to say with the info, but I would guess not enough light and it is dying. It is stretching out to get the light Also what is your water parems and how old is your tank? Has the nem moved in the tank recently?

My understanding is they have a tendency to just not go back to the bubble look in out tanks. It may every once in a while but the extend their tentacles as they need them to work. As long as it is healthy and keep feeding there is nothing to worry about.

You might want to add another 2 bulb T5. Use 10000K bulbs in it.

+1 on this. In some tanks, you'll get bubbles, in others, nada.

All of my BTA's were bubble tips and after being in my tank for a couple of weeks, they lose their bubbles. Now they look like LTAs.

All of my nems are doing fine though, so don't worry unless the tentacles start retracting and stay retracted or if the 'nem won't stay attached.

Either way on the light, don't think the clown bit the nem but anything is possible with clowns.
