Have you checked your ammonia and nitrite levels lately? Maybe you were going through a mini cycle and didn't even know it.

Have you added anything new to the tank recently? I just had a few fish die on me as well. I tested and all I did was add one fish. I tested my levels and my ammonia and nitrites were elevated. I took care of it though, and everything is just fine now.

What I mean by mini cycle is just a shorter version of the original cycle your tank goes through when set up initially. The first cycle can take anywhere from 4 weeks to a couple of months to finish. A mini cycle takes a shorter amount of time because your system is already established. Your bacteria cultures just need to catch up in order to keep up with the heavier bio load.

Could also just be that he/she was old. We just never know the true age of store bought/wild caught fishes.

That is a freshwater fish. It is almost imposible to tell how a fish has died by looking at it. You would need to do a necropsy on it. Sorry for your loss.
