change your filter media or pads too when was the last time you cleaned out filters and skimmer? Like a good clean? I would do a bunch of smaller(4-5 gal) water changes within the next few days. helps me

I agree, water changes and cleaning all your filter media. The other thing I would suggest is to relax. If you have only lost one fish and the rest of your livestock looks good, like your corals are all still opening up and your fish look well, then you are probably in no immediate danger. Also make sure your testing kit hasn't expired. You could be getting a false reading.

At first glance I am going to guess you feed a lot since you have 12 fish. You also only have 40 to 50 LBS of LR, you should have 75 minimum. How often do you do water changes? What do you feed?
