Well...I seem to have the same type issue.
All was well at 6:00am...noticed at 9:30am ..the powder blue with heavy breathing..and wanting to settle in one place and just resporate rapidly....still doing so....CHANGES= I added a better powerhead yesterday and incresed the water flow(replaced a Fluval2 with a 400gph directional type powerhead)...but the tank overall responded favorably to it.
Other than that all my params are good...
This would be my first loss...and it dont feel good.

Sorry for your loss man. I just lost my Kole tang the other day as well. Very cool fish, but I'm not so sure I will get another now that this seems to be a reaccuring issue.
