Agreed, flatworms. Here are some ways to get rid of them;
1. It is siad that a sixline wrasse will eat them, but the trick to that is you have to under feed it which is highly impossible unless he's the only fish in the tank.
2. I have also used a small turkey baster and take them off one by one( I know )
3. Salifert's Flat worm exit( I havent used that yet)
4. Doing a FW dip for 1-5 min.s
5. There might be a "old school" method, but I dont know of any. Might want to PM Tangster.
