Years ago I had 3 Oscars (Red, Tiger, and Alino). I think i remember the hole in the head disease was caused by parasitic worms in the tank (and in the fish). The worms don't do much harm until they eat in far enough and start munching on the Oscar's brain. It could go quick or it could last awhile. I think I gave mine some parasite treatment and they lasted over 3-4 years until the outgrew 8"+ the 55 gallon tank. I moved to my own house and now it's a new SW tank.
